Terms of Service

By creating an account or using the Forma mobile application or website at formaapp.health ("the Service"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use the Service. These Terms constitute a legally binding agreement between you and Forma Health Ltd ("Forma", "we", "us", "our").

You must be at least 18 years old to use Forma. By using the Service, you confirm that you meet this requirement. If you are under 18, you may not create an account or use the Service.

Forma is an AI-powered personal health coaching application. The Service delivers personalised daily instructions covering hydration, nutrition, exercise, sleep, and cycle health, based on information you provide and data from connected apps such as Apple Health and your calendar. Forma is a coaching tool, not a medical service. The instructions and content generated by Forma do not constitute medical advice, diagnosis, or treatment. You should always consult a qualified healthcare provider for any medical concerns, and before making significant changes to your diet, exercise routine, or health regimen.

You are responsible for maintaining the security of your account credentials. Do not share your password with others. You are responsible for all activity that occurs under your account. You must provide accurate information when creating your account. Providing false or misleading information may result in termination of your account. If you believe your account has been compromised, notify us immediately at support@formaapp.health.

Forma is available on a subscription basis. Pricing and plan details are set out on our pricing page at formaapp.health/pricing. Free trial: New subscribers receive a 7-day free trial. You will not be charged during the trial period. If you cancel before the trial ends, no charge will be made. If you do not cancel, your subscription will begin automatically at the end of the trial. Billing: Subscriptions are billed monthly or annually, depending on the plan you select. Payments are processed by Stripe on behalf of Forma Health Ltd. By subscribing, you authorise us to charge your payment method on a recurring basis until you cancel. Cancellation: You may cancel your subscription at any time via your account page at formaapp.health/account. Cancellation takes effect at the end of the current billing period. We do not provide refunds for partial periods, except as required by applicable law. Price changes: We will give you at least 30 days' notice of any price changes. Continued use of the Service after a price change takes effect constitutes acceptance of the new price. Subscriptions are managed directly through formaapp.health, not through the Apple App Store. No App Store commission is added to your price.

Forma provides coaching content generated by artificial intelligence. This content is for informational and motivational purposes only. Forma is not a medical device. It is not regulated by the FDA or any other medical regulatory authority. Nothing in the Service is intended to diagnose, treat, cure, or prevent any disease or medical condition. If you have a known health condition, are pregnant, are recovering from illness or injury, or are taking medication, consult your doctor before following any instructions provided by Forma. Stop using Forma instructions and seek medical attention if you experience pain, discomfort, or any adverse reaction.

You agree not to: — Use the Service for any unlawful purpose or in violation of any applicable law. — Attempt to gain unauthorised access to any part of the Service or its underlying systems. — Reverse engineer, decompile, or disassemble any part of the Service. — Use automated scripts, bots, or scrapers to access or interact with the Service. — Impersonate any person or entity or misrepresent your affiliation with any person or entity. — Submit false, misleading, or harmful information to the Service. — Resell or redistribute the Service without our express written permission. We reserve the right to suspend or terminate accounts that violate these terms.

All content, features, and functionality of the Service — including but not limited to software, text, graphics, logos, and the Forma coaching methodology — are owned by Forma Health Ltd and are protected by copyright, trademark, and other intellectual property laws. You are granted a limited, non-exclusive, non-transferable licence to access and use the Service for your personal, non-commercial use. You may not reproduce, distribute, modify, or create derivative works of any part of the Service without our express written permission. Content you generate within the Service (such as your onboarding conversation and responses) remains yours. By submitting it, you grant Forma a licence to use it to provide and improve the Service.

Your use of Forma is also governed by our Privacy Policy, available at formaapp.health/privacy. By using the Service, you consent to the collection and use of your information as described in that policy.

Forma integrates with third-party services including Apple Health, Apple Calendar, Stripe, RevenueCat, Anthropic, OneSignal, and Supabase. Your use of those services is subject to their own terms and privacy policies. Forma is not responsible for the practices or content of third-party services.

To the fullest extent permitted by applicable law, Forma and its directors, employees, and service providers shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or related to your use of the Service, including damages for personal injury, loss of data, or loss of revenue. Our total liability to you for any claim arising from or related to the Service shall not exceed the amount you paid to Forma in the 12 months preceding the claim. Some jurisdictions do not allow the exclusion or limitation of certain damages, so some of the above may not apply to you.

The Service is provided "as is" and "as available" without warranty of any kind. We do not warrant that the Service will be uninterrupted, error-free, or free of harmful components, or that any errors will be corrected. We make no representations about the accuracy, reliability, or completeness of the health coaching content generated by the Service.

We may suspend or terminate your access to the Service at any time, with or without cause, with or without notice, including if we believe you have violated these Terms. You may delete your account at any time by contacting us at support@formaapp.health. Upon termination, your right to use the Service ceases immediately.

These Terms are governed by and construed in accordance with the laws of England and Wales, without regard to conflict of law principles. Any dispute arising from or related to these Terms or the Service shall first be attempted to be resolved through good-faith negotiation. If unresolved after 30 days, disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.

We may update these Terms from time to time. When we make material changes, we will notify you by email at least 14 days before the changes take effect. Your continued use of the Service after the effective date constitutes acceptance of the updated Terms. If you do not agree to the revised Terms, you must stop using the Service and cancel your subscription before the effective date.
